SUBJECT : COMPLIANCE TO THE ISSUANCE OF IDENTIFICATION CARDS AND PURCHASE BOOKLETS FOR
PERSONS WITH DISABILITY (PWDs)

Republic Act 9442, the act amending R.A. 7277, otherwise known as the Magna Carta for Disabled
Persons and for Other Purposes, provides priviledges, incentives and other benefits to persons with
disabilities.
To ensure that persons with disabilities (PWD) will fully enjoy the benefits and privileges provided for in
RA 9442, the National Council on Disability Affairs issued NCDA Administrative Order No. 001 series of
2008 providing the mechanisms for the issuance of Persons with Disabilities Identification Cards (PWDIDC) and NCDA Administrative Order 003 providing for the guidelines for the computerization of the
issuances of ID cards.
In this regard, we enjoin all Local Government Units to facilitate the issuance of the identification cards
and purchase booklets to all PWDs in their respective jurisdictions, guided by the following procedures.
1. Provision of the registration form to PWDs (attached is teh PWD-RF).
2. Process accomplished form, check and/or verify documentary requirements.
3. Issue PWD-IDs and purchase booklets signed by the Mayor. (ID and booklet sample attached)
4. Retain all approved PWD-RFs for encoding in the PPWDRS of DOH.
Likewise, all DILG Regional Offices are directed to cause the widest dissemination of this Memorandum
Circular.
